Product Certification Application Form (FR14)

You can submit your product certification application by filling in the form below. When you submit the form, your information is generated as a PDF in the format of FR14 Product Certification Application Form, sent to TESTPLUS, and a copy is sent to your e-mail address.

  1. Fill in the form and press the Submit Application button.
  2. Print the FR14 PDF form sent to your e-mail address (or downloaded from this page) and send it to TESTPLUS stamped and signed.
  3. Attach your technical file, product catalogue, company registration documents and, if any, your trademark registration certificate to your application.
  4. Your application is evaluated as soon as possible and the offer is sent to you in writing.

    Undertaking

    I have read the TESTPLUS Product Certification Application Form and undertake to comply with its requirements. I declare that all the information given above is current and correct, and I accept responsibility for any adverse consequences arising from missing or incorrect information.

    I declare and undertake that, as of the date of completing this form, I have not applied to any other conformity assessment body or notified body for the products mentioned in this form with the same technical file.

    I undertake to fulfil the requirements of the approved quality system for the products I manufacture and to maintain it fully and effectively.

    If I hold a certificate from another notified body for the products covered by this application, I undertake to provide and share with TESTPLUS the notified body that issued it, the date of certification, information on surveillance audits and the results of previous audits.
